| Pre-Exam Patient Questionnaire - Online Version | If your cat will be visiting us for the first time, or if your cat has not been to see us within the past 6 months, and/or is ill, we ask that you complete our "Pre-Exam Patient Questionnaire". This can be filled out online and sent to us electronically. It can also be downloaded, printed, completed and brought with you to your appointment, or it can be filled out at our hospital before your appointment. (Be sure to arrive 15-20 minutes prior to your scheduled appointment time if you choose the last option.)
